Information for patients wishing to register with a

GP practice at Breeze Hill Neighbourhood H/C

There are 3 GP practice’s within the Health Centre below you will find general information about each practice to help you choose which GP Practice you wish to register with. If you require more information about an individual practice you can request a practice leaflet from that practice. Each GP practice has a practice boundary and registration with the GP practice of your choice is dependant upon your current address and if it falls into their boundary.

N.B

Families living at the same address must be registered with the same practice.
Patients who find dissatisfaction with a particular practice or GP can not assume they can automatically register with another practice within the building they should discuss their concerns with their practices Practice Manager
All NHS establishments have a zero tolerance policy; patients who behave abusively or violently towards any member of staff within the Health Centre will be removed from the practice list; patients removed under zero tolerance will be unable to re-register with another practice within the Health Centre.

With special interests in:

Child Health, Vacc & Imms including Travel Vaccs, Chronic Disease Management, Acupuncture, Joint Injections, Cryotherapy (wart clinic), Dermatology and Sexual Health

We have appointments available in advance up to 4 weeks and appointments available on the day. All urgent requests will be dealt with on the day of request.

Opening Hours: 08:00 – 18:30 Monday to Friday (Phone lines from 08:00 reception from 18:30)

Clinic Hours: 09:00 – 12:30 14:00 – 18:00 Monday to Friday

WALTON MEDICAL CENTRE

Has 4 GPs, 3 male and 1 female as well as a full complement of nursing and administrative staff.

Our opening hours are 8.30am – 6.30pm Monday to Friday and we offer bookable appointments at various times throughout the day from 8.30am until 5.30pm.

We offer the following services in addition to the usual morning and afternoon GP sessions:-

Joint injections, cryocautery i.e. wart/verucca clinic and childhood immunisations
