Outstanding Staff Contribution Award
Guidance
Purpose:
‘Outstanding contribution award’ recognition is not just a nice thing to do for people, it is about recognising and sharing good practice and outstanding contribution within the workplace and continuing to develop a person centred culture within Future Directions CIC.
Mission Statement:
‘Enabling individual choice and control to achieve fulfilled lives’
Criteria:
All employees are eligible.
Anyone can nominate another person at any level of the organisation.
Employees can receive the reward more than once.
Employees must demonstrate initiative, exemplary individual achievement and contribution in their work, showing outstanding performance in their job role.
The following criterion should be considered:
How has the employee made a real difference to a person / people they supports life? (Choice and control / community participation / family and friends)
How has the employee demonstrated the person centred ethos of community services?
How has the employee made the effort to go the extra mile within their workplace?
How has the employee excelled in their performance?
How has the employee promoted teamwork and inspired excellence in others?
The process:
Any individual can complete a nomination form, for example: - staff member, service user, family member.
Forms will be accepted at any time and must be submitted to Kim Barrett (Person Centred Planning Co-ordinator)
Each nomination form will be held for 90 days, so that if more than one extemporarily nomination is received within the nomination period, it can be relooked at the next nomination meeting.
The nomination form will be taken to the Future Directions CIC operation meeting every three months; January, April, July, October. The meeting is usually the first Tuesday in the month.
The employee will receive:
A ‘Holiday Voucher’ entitling them to one day’s annual leave.
A letter from Director of Operations Paul Lord
A certificate and a photo of them receiving the certificate.
Recognition in Future Directions CIC’s newsletter.

How has the nominee demonstrated outstanding performance within their job role? Gone the extra mile. Extra evidence can be attached, for example a complete achievement tool, letters of recognition.
