QUALIFYING MEDICATION ERRORS
(Addition to PAMP, Perfected Approach to Med Pass) 09/11/18
The Wisconsin Bureau of Assisted Living’s Medication Management Initiative Workgroup defines a medication error as follows: A medication error is a preventable event resulting in the incorrect administration of a medication, or harm or potential harm to a resident. An error occurs when the resident does not receive a medication as prescribed/ordered by the physician.
Purpose: The purpose of qualifying a major or minor medication error is not to demean its significance or potential for harm, as any error great or small, holds the potential for an adverse resident event. It should be noted that a ‘minor’ error has the potential to turn into a ‘major’ error. At this point, qualifying medication errors is for the purpose of gathering data and statistical tracking for IM’s Key Performance Indicators/Scorecard.
