1. Welcome

Practice Manager welcomed Jan Donachie to the Group.

2. Minutes of previous Meeting

The minutes of the meeting held on 19th March 2014, were agreed.

3. Patient Survey 2014

Practice Manager advised that the welcome pack had now been completed and gave copies to the Group for comment. Issue to new patients would be on the basis of one per household.

Generally, the group agreed that the pack provided valuable information about the practice but commented that some of the information sheets were not printed/cut properly. This would be corrected in future packs. It was also agreed that the Practice information sheet, which was updated on a periodic basis, would show the issue date, thus allowing patients to check they have the current issue.

4. Practice Update

a) Allocation of named GP

Practice Manager advised that as part of the Government's enhanced services initiative, letters have been sent to those patients aged 75 and over advising the name of their allocated GP within the practice.

b) Unplanned Hospital Admissions

Practice Manager stated that as part of the measures intended to reduce unplanned hospital admissions, the target of 2% high risk patients within the practice had been identified. These were those patients seen as being at high risk of an unplanned hospital admission. Letters had been sent to those patients with a view to building with their GP, a health care plan which would, hopefully, reduce the risk of them being admitted to hospital on an unplanned basis.

Group also noted arrangements, under the name “Hospital at Home”, whereby patients were allowed home from hospital but remained under hospital care. Treatment was provided by a nurse from the hospital visiting the home. This was however a limited arrangement and only for those patients where this was considered a suitable means of care.

c) Health Care Assistant vacancy

Practice Manager advised that this vacancy had been filled. Some training (ear syringing) was still required but overall, it was found that the successful applicant was well suited for the requirements of the role. The part time receptionist vacancy had also been filled, also with a very satisfactory candidate.

5. Patient /Community News & Issues

a) Closure of Wyvill Practice

Practice Manager advised that following the sudden death of the single practicing GP at the Wyvill Close, Parkwood, that the practice had now closed. As a result, some 3,700 patients needed placement with an alternative GP. Approximately 300 of those patients had selected to transfer to this practice and they had been issued with the “new patient” pack. It was noted that patients from the Wyvill Close practice were not all in the immediate area and it was not thought that any further significant number would be joining this practice.

6. Any other Business

a) PPG Open Day

Agreed that a Patient Participation Group open day should be held again this year. As previously, volunteers from the Group would discuss with patients, the group/issue leaflets within the practice waiting area. A suitable date to be agreed.

b) Test Results

Responding to a suggestion that patients be contacted by the surgery when test results were received, Practice Manager explained that the large number of test results received each day made this impracticable. However, patients were contacted if a Doctor felt it necessary to discuss results with the patient.

c) Resuscitation Training

Agreed that training for only a very limited number could be undertaken within the practice. However a Group member would contact the Medway Asthma Self Help Group where a larger room may be available.

7. Date of next meeting

Agreed the next meeting would be on Wednesday 17th September 2014 commencing at 6.30pm.
