  1. Each school has a duty to ensure that it carries out its responsibilities under Health and Safety during any alterations or improvement to its premises. One of the following processes must be adhered to. The process followed will depend on whether or not the Structure or Fabric of the Premises is affected.

  1. The School should informally discuss its proposal with JLIS and if the proposal is feasible, Section 1 of this form must be completed. The School should attach any information that it has – specification, drawings etc. to enable the process to move forward with as little delay as possible.


  1. The School must forward the above information by email to [email protected] or send via internal post, addressed to: PFI Team, Room 1.13, Sanford House.


  1. Children’s Services will complete Section 2, and raise either a Small Works Order or a Variation (ANVR), which will be forwarded to ESSL /JLIS for comment and costing (20 working days).


  1. ESSL / JLIS will complete Section 3 of this form and provide the following information, if applicable:






  1. ESSL / JLIS will forwarded the above information by email to [email protected] or via post to PFI Team, Room 1.13, Sanford House, Sanford Street, Swindon, SN11QH.


  1. Children’s Services will inform the School of the costings and forward a hard copy of all the information (plans / specification) provided by ESSL / JLIS to Property Services at SBC to request authority to proceed (10 working days).


  1. Once Section 4 of the form has been completed by Property Services and subject to approval, Children’s Services will issue the instruction to proceed (or not) to ESSL / JLIS.


  1. JLIS will liaise with the Head Teacher / Business Manager to ensure that all processes have been followed as detailed on Health & Safety Roles & Responsibilities – Works Undertaken on PFI School Sites flowchart.
