Examples of body of Modification Agreement:

The document attached to this agreement and entitled "Revised Schedule A to the Service Agreement between Her Majesty the Queen in right of the Province of British Columbia and (Contractor's name)" replaces the Schedule A attached to the Agreement, effective (DATE).
Section 3 of Schedule B of the Agreement is deleted and the following substituted: (PROVISION)
The term of the agreement is extended to end on (DATE).
