Background and purpose
This Activity Notice relates to the insert Activity name (the Activity). Include a brief description of the Activity.
The purpose of this Activity Notice is to provide adequate information to assist YSRC to make an assessment as to whether a Survey is required in relation to the Activity and, if so, whether a Site Identification Survey or a Site Avoidance Survey is required, and, if a Survey is required, to provide information relevant to the conduct of a Survey. The inclusion of details at (c) to (f) below does not prejudice the statement at (b) below that the Proponent considers that a Survey is not required [Delete this sentence if the Proponent indicates at (b) that a Survey is required].
Key statements and nominations under Clause 8.2(c)(ii) and Part 1 Clause 1.2 Schedule 4 YPSHA
The Activity Program consists / does not consist of Low Ground Disturbance Activity, in the opinion of the Proponent – where the Activity Program consists of Low Ground Disturbance Activity, include a statement of the extent of that activity in the relevant area.
The Proponent considers that a Survey is required / is not required – (taking into consideration matters referred to in clause 8.1(b) and 8.3 and setting out those relevant matters with respect to this particular Activity.
Possible Reasons for notification that an Aboriginal Heritage Survey is not required may include, but not be limited to:
the Proponent believes a survey is not required, but is unsure as to whether clause Error: Reference source not found operates to exempt it from giving an Activity Notice, and is issuing one to seek YSRC advice
area has been previously surveyed
site information in the areas is reasonably known and works will not impact any sites
area is heavily disturbed
supplementary activity to previous Activity Notice,
works within the footprint of:
existing infrastructure (building)
existing infrastructure (easement) :.
The Proponent’s preferred Survey Methodology is a Site Identification Model / Site Avoidance Model
where a Site Avoidance Model is nominated, a statement of whether the Proponent requires any Survey to be conducted in respect of:
only the area or areas to be impacted by specific Activities as described and mapped in the Activity Notice (commonly known as a 'work program clearance' survey); or
a broader area or areas, encompassing the Activities and surrounding land as described and mapped in the Activity Notice (commonly known as a 'work area clearance' survey); – where a Site Avoidance Model is nominated a statement as per clause 1.2 (d) Part 1 Schedule 4 YPSHA is to be added.
The proposed Survey fieldwork start / end date is date.
The Proponent requires:
Preliminary Advice (see clause 12.1(a) YPSHA
a Draft Survey report (see clause 12.1(b) YPSHA);
Where any of these key statements are omitted, Part 3 of Schedule 4 YPSHA provides default provisions.
This Activity notice also includes:

In accordance with clause 8 of this YPSHA, the purpose of the information provided in and with the Activity Notice is to determine whether a Survey is required and if so, its nature and extent. In order to facilitate this objective, an Activity Notice must contain the following additional details where applicable:
Attachment 1: Map showing the area and digital spatial data the subject of the Activity Notice in accordance with YPSHA Clause 8.2(c)(iii) and Part 2 Schedule 4
Attachment 2: Aerial photographs (if available) or smaller maps showing the area subject of the Activity Notice in accordance with YPSHA Clause 8.2(c)(iii) and Part 2 Schedule 4
Attachment 3: Activity Notice Program information in accordance with YPSHA Clause 8.2(c)(iii) and Part 2 (d) through (j) Schedule 4
Attachment 4 (OPTIONAL) Notification that the Proponent considers it may be necessary to lodge a s16 or a18 Application under the Aboriginal Heritage Act 1972 (AHA) provided in accordance with Clause 10.2(d)(iii)(F) and a requirement that the survey must record sufficient information to address Department of Planning Lands and Heritage (DPLH) requirements for such an Application.
Note: This attachment does not replace the need for the Proponent to provide YSRC with a ‘Notice of Application’ as required by Clause 17 if following the survey the Proponent decides it still intends to lodge a s16 or s18 Application.

ATTACHMENT 4: (OPTIONAL)
Notification that the Proponent considers it may be necessary to lodge a s16 or a18 Application under the Aboriginal Heritage Act 1972 (AHA) provided in accordance with Clause 10.2(d)(iii)(F) and a requirement that the survey must record sufficient information to address (DPLH) requirements for such an Application.
Information on whether the Proponent may be necessary to seek a s16 or s18 Application under Aboriginal Heritage Act 1972 (AHA), including provision of all advice required to lodge an Application, including information as set out in 10.2(d)(iii) and 12.4 and Schedule 6 of the YPSHA.
Note: This attachment does not replace the need for the Proponent to provide YSRC with a ‘Notice of Application’ as required by Clause 17 if following the survey the Proponent decides it still intends to lodge a s16 or s18 Application.
Attachment 5: OPTIONAL
A written copy of previous Aboriginal Heritage Survey Reports, in accordance with YPSHA Clause 8.2(c)(iii) and Part 2 (a) ‘An Activity Notice may also set out’ in Schedule 4
whether there has been any previous Aboriginal Heritage Survey and, subject to any confidentiality restrictions, the age, methodology, participants, standard and results of that survey. If a written report of that previous Aboriginal Heritage Survey is in the possession or control of the giver of the Activity Notice, then (subject to confidentiality provisions) the Activity Notice shall be accompanied by a copy of the written report, including any Closed report for which permission is required to be sought from the informants for disclosure. That permission must be sought before the Activity Notice is issued so that the written report can be provided to the Corporation;
ATTACHMENT 6: OPTIONAL
Additional Activity Notice Program information, in accordance with YPSHA Clause 8.2(c)(iii) Part 2 (b) through (e) ‘An Activity Notice may also set out’ in Schedule 4
the extent to which the area the subject of the Activity Notice has been affected by previous ground disturbing activities;
whether the Aboriginal Heritage Act Register discloses any Aboriginal Sites on the area the subject of the Activity Notice;
any additional information which explains what sort of Survey outcome is being sought (if a Survey is required); and
any other background material which will better help the Corporation and the Traditional Owners to understand the potential impacts of what is proposed.

ATTACHMENT 7: OPTIONAL
Information on area, terrain and environment of land proposed to be inspected during the heritage survey in accordance with YPSHA Part 4 Schedule 4
Information on area, terrain and environment of land proposed to be inspected during the heritage survey that is relevant to the physical demands of the survey, and which could be relevant considerations in the nomination of Aboriginal Consultants.
Expected schedule of activities and associated timeframes, including lunch and other breaks (indicative only)
Mode/s of transport (eg by foot, car, plane), and expected ratio between different modes of transport

Where temperatures are forecast to exceed 40 degrees Celsius this information must be included in any Activity Notice which involves surveys between the days of 1 December to 1 March.
Where conditions exist or are reasonably anticipated which the Corporation would be required to comply with under the provisions of any applicable legislation relating to occupational health and safety.
Any other factors that may prevent particular individuals from fully participating in all aspects of the survey (eg where individuals have restricted mobility, medical conditions, etc).
