Purpose
To review accidents that occur on the Spring Grove Hospital Center campus when an employee is driving a State vehicle (licensed or unlicensed). Although the Accident Review Board does not have jurisdiction over employees driving their own vehicles, who may hit a State vehicle or State property, such accidents could be investigated by the Spring Grove Hospital Center Police Department and the employee subject to drug testing.
Policy
The Accident Review Board will review every vehicular accident, as described above within thirty (30) days from the date of the occurrence and make recommendations to the COO.
Organization
The Accident Review Board will be structured and organized as follows:
Chairperson appointed from Safety Committee membership
Chief Finance Officer
Chief of Police
Director, of Environment of Care
Fleet Safety Coordinator
Representative from Human Resources
All members of the Accident Review Board are appointed by the Superintendent/CEO and must sign a statement of confidentiality before beginning service on the Accident Review Board.
The Accident Review Board will meet monthly at a time and day stipulated by the Chairperson unless there are no vehicle incidents to review.
Recommendations from the Accident Review Board shall be based on a majority vote of those present. A quorum will consist of any four (4) of the six (6) members including the Chairperson.
The Fleet Safety Coordinator’s responsibilities will be as follows:
Serve as a member of the Accident Review Board.
Assure that only staff that have current Driver Improvement Training (DIP) are permitted to drive State vehicles.
Coordinate insurance claim with the SGHC Chief Finance Officer and the State Treasurer’s Office Insurance manager for SGHC vehicle accidents.
Ensure and coordinate the appropriate completion and distribution of the following forms:
State of Maryland Automobile Loss Report Form. This form must be completed and distributed within forty-eight (48) hours of the accident.
Motor Vehicle Accident Investigation Guide, FS-1 Form, page 1. This form must be completed within forty-eight (48) hours by the driver and FS-1 page 2, within seventy-two (72) hours by the driver’s supervisor.
Copies of each form are to be sent to the State of Maryland Insurance Division, SGHC Human Resources, and the SGHC Accident Review Board Chairperson.
The SGHC Police Department will investigate every accident and provide a report to the Accident Review Board Chairperson. As part of the report, a copy of the employee’s driving record will be obtained and provided to the Accident Review Board Chairperson.
The Accident Review Board is a sub-committee of the Safety & Infection Control Committee. The Chairperson of the Accident Review Board will provide quarterly reports to the Safety Committee. These reports will be written and will not list names of the employees involved in accidents to maintain confidentiality.
Procedure
The Spring Grove Hospital Center Accident Review Board will operate as follows:
The Board will review SGHC vehicle accidents to determine:
Driver history and performance
Accident cause(s) and conditions
Accident preventability
Recommended driver corrective action(s)
The Board shall provide a summary of findings and recommendations to the SGHC COO. If the accident is found preventable, the Board shall recommend driver corrective actions(s). Driver corrective actions can include any combination of the following:
Verbal Counseling
Attending a Driver Improvement Program prior to being permitted to drive State vehicles or equipment.
Written Reprimand, including a statement of facts concerning the accident, to be part of the employee’s personnel file.
Mandatory attendance at a State approved MVA Driver Improvement Training Program for employees who have had two or more accidents in a rolling twelve month period, for which they were at fault. This program cannot be internet based.
Evaluation by Concentra to determine if there is a medical reason impacting driving ability.
Damage assessment for actual damages to State-owned property or vehicles may be levied up to $100.00 maximum based on repair estimates.
Suspension of driving privileges at SGHC for six (6) months to three (3) years.
Suspension of duties without pay for a length of time recommended by the Accident Review Board.
The COO shall review the Accident Review Board findings and recommendations. Approved recommendations shall be returned to the appropriate supervisor for action.
Employees will have the right to appeal the recommendations of the Accident Review Board. Such appeal must be submitted to the COO in writing within thirty (30) days of the employee’s receipt of the corrective findings.
