REPEAT PRESCRIPTIONS

If you would like a further supply of your current medication, you can make a request via the Surgery website, however, you will need to register before you can use this on-line service alternatively please either fill in a “repeat prescription request slip” (ask Reception) or complete the white right- hand side of an existing prescription.Please always indicate the chemist of your choice to dispense the medicine, otherwise prescriptions will be kept at the Surgery awaiting collection. We are unable to accept requests for repeat prescriptions over the telephone except for our housebound or very elderly patients. PLEASE ALLOW TWO WORKING DAYS for the Doctor to deal with your prescription requests.


MEDICAL CERTIFICATES

For your first week off work you do not require a Doctor’s certificate. If your employer requires a ‘sick note’ from your Doctor this can only be provided in the form of a Private Medical Certificate for which there is a charge. After the first week off work, your Doctor can complete a Statement of Fitness for Work or “fit note”.

ACCESS TO MEDICAL RECORDS

We are compliant with GDPR (General Data Protection Regulation) which came into force on 25th May 2018 and governs how we collect, store, use and share your personal information. We take patient confidentiality very seriously. For any requests by third parties to access your medical records, your written consent will be required. For further information please refer to our data protection privacy notice on our website and a copy is available to read in the waiting room.


THE FRIENDS OF JUDGES CLOSE SURGERY

The Practice enjoys a good relationship with this volunteer Patient Participation Group. We view the group’s existence as a channel for two-way communication between the Practice and the patients, for mutual benefit. If you would like to join this group, please contact HSCCG.Judgesclose-Eastgrinstead.nhs.net or ask at reception.


PARKING

We have limited parking space at the Surgery but it is usually sufficient providing patients remove their cars as soon as they have completed their business at the Surgery. We realise it is tempting to ‘pop into town’ but this does cause congestion which is particularly distressing to the elderly and disabled patients who are then unable to park.


SPECIAL NEEDS / DISABLED ACCESS

All our patient services take place on the ground floor and there is wheelchair access around this area, including toilet and baby-changing facilities. We have a portable Induction Loop for hearing aid users. Please ask at Reception if you would like this to be used during your consultation. If you have a special need of any description and experience difficulty in communication or access within the Surgery, please contact any member of the team

STATEMENT OF PURPOSE

We are a GMS Practice and our health professionals diagnose, treat and manage illnesses and act as a patient’s advocate ensuring they receive a high standard of care. We provide the link to;


  • further health services

  • help patients to manage their health

  • arrange hospital admissions and referrals to hospital outpatients and community services



We also collect and record important information from other healthcare professionals involved in the treatment of our patients. Our Health Professionals are also involved in the education and training of medical students, practice staff and other healthcare professionals. We value all patient feedback and encourage involvement with our Patient Participation Group.


THE PRACTICE MANAGER AND STAFF

Mrs Christine Earwaker is our Practice Manager, she is responsible for the day-to-day running of the Surgery and is supported by clerical, administrative and secretarial staff.

RIGHTS AND RESPONSIBILITIES

Everyone working at the Practice is bound by a legal duty to maintain the highest level of confidentiality. We will treat you as an individual and respect your privacy and dignity. In return, we ask you to be polite to all our staff. Violent or abusive behaviour, whether actual or threatened, will not be tolerated and may result in your removal from the Practice list.


Please do try to keep appointments, and tell us quickly if you are unable to do so. If you arrive after your appointment time this may cause delays and inconvenience to other patients. If you are more than 10 minutes late, you may have to rebook. If we are running late please be patient – on another occasion, it may be you who needs the extra time.


NEW PATIENTS

All newly registered adults who are already on medication, are eligible for a registration medical check with a GP. If you are not on any medication we advise you to make use of the ‘surgery pod’ in the waiting area to records details onto your medical record.


Patients are registered with the Practice and not an individual GP, however you can still ask to be seen or treated by a particular practitioner. Every effort will be made for you to be treated by the Doctor of your choice. However, this will be dependent upon your request being appropriate, reasonable, and that the Doctor requested is available.


YOUR PERSONAL DATA

The Practice is required to supply personal and confidential medical information to the Health and Social Care Information Centre (HSCIC). We are legally bound to allow the HSCIC to extract this information from our records, however individuals do have the right to stop the transfer of their data. If you do NOT wish your information to be shared, please request an ‘opt out’ form for completion from Reception.



URGENT ON THE DAY APPOINTMENTS

Any patients ringing in the early morning requesting an urgent appointment will be asked by the Receptionist the reason for the appointment. This is to assess which health professional is most suited to deal with your problem. All patients requiring medically urgent care will be offered a same day appointment.


ROUTINE APPOINTMENTS

Appointments for routine or non-urgent consultations will be booked by a Receptionist at a mutually agreed date and time. It is also possible to book appointments on-line through the Patient Access booking system after having registered for this service. Please ask a Receptionist for details – you will need to have photographic ID to obtain the on-line registration login details.


For ROUTINE appointments or queries about results please telephone the Surgery after 10am.


TELEPHONE ADVICE

The Doctor or Nurse is available to speak to you on the telephone for advice rather than face to face. You can request such an appointment when you speak to the receptionist.

HOME VISITS

If you are unable to travel (housebound, elderly or infirm) we are able to offer home visits. Please try to make the request before 12 noon and give the receptionist details of your illness, in order for the Doctor to determine priorities.


Our Receptionists are not medically trained but are bound by the same rules of confidentiality as the Doctors and Nurses. In some cases the Doctor will telephone before making a home visit to gather more information or offer advice.
