Note: The average processing time for MediSave accreditation process is 40 working days from the date CPF Board receives the full set of documents. The process may take longer should there be any delay between steps 2 and 6.

Documents Required For A Private Limited Company
A1 - Board Resolution – Preparation Guide
Please refer to the template of the Board Resolution at page 6, which a Private Limited Company will need to complete and submit to CPFB.
You will need information from the company’s ACRA BizFile; Memorandum and Articles of Association/Constitution; and the MOH License, to help you complete the Board Resolution.

If a majority of Directors are required to sign the Resolution, please ensure than more than 50% of the Directors as updated in the ACRA BizFile, sign the Board Resolution.

D1 - Bank Forms Required by CPF Board
You will need to complete a total of TWO bank forms as part of your Medical Institution (MI)’s participation under the MediSave/MediShield Life Schemes. They are as follows:

Medical Institutions may submit the required documents (except for the DDA bank forms as original forms are required by your bank) to CPF Board according to their business entity via CPF Board’s My Mailbox service by logging in with the clinic’s SingPass / CorpPass after submitting your accreditation application with MOH.

Upon clicking the link “Access Online Application for Business Partners” as shown above, you will be redirected to the SingPass / CorpPass login page. Please login with your SingPass / CorpPass details and you will be able to assess CPF Board’s Business Partners Online Services.

After you have login successfully, you will be able to assess the webpage below, Click on “My Mailbox” as shown via the blue arrow below and you will be able to see the “New Enquiry” button. Click on the “New Enquiry” button as shown below.
After clicking the “New Enquiry” button, the following screen will appear:

Lastly, click on the “Submit” button above and the document submission process is completed.
8. Check List
Before submitting your documents to CPFB, please refer to the Check List to ensure that you have all the required documents properly completed and prepared.
