Make all school checks, booster checks, or money orders to: NC Music Educators Assn. Checks must accompany forms. Do not send checks under separate cover and DO NOT SEND A PERSONAL CHECK. Personal checks will not be accepted. Put your school name at the bottom of the money order.

Fill out one form per student. Read this information carefully. Some did not follow this last year.

Use a brown clasp envelope for the forms. Please use enough stamps to get them mailed. Do NOT fold any of the forms.

Please staple the medical and honors rules sheets together and alphabetize them by last names.

Please PAPERCLIP the registration forms together and ALPHABETIZE by last name.
