DISABILITY VERIFICATION GUIDELINES FOR STUDENTS WITH PSYCHIATRIC DISORDERS


PLEASE TYPE RESPONSES TO QUESTIONS IN A SEPARATE REPORT



_________________________________________ has requested accommodations from The University of Arkansas for a psychiatric disorder. To determine any necessary and reasonable accommodations, the University needs specific information about how the student’s psychiatric disorder constitutes a substantial limitation to a major life activity. Toward that end, we request that you provide the following information in a typed report on letterhead.


  1. Diagnostic criteria:

  1. Diagnosis by licensed psychologist, psychiatrist, licensed counselor, or clinical social worker that is an impartial individual not related to the student.

  2. DSM-IV designation.

  3. Date of diagnosis that is no older than three years.

  4. Last contact with student.


  1. Describe symptoms that meet the criteria for this diagnosis.


  1. Summarize present symptoms and prognosis.


  1. Identify the major life activities affected by this disorder such as walking, learning, seeing, hearing, sleeping, etc.


  1. Is the student currently taking any medication related to the condition for which the student is asking for reasonable accommodations? If so, please relate the impact of that medication on the student’s ability to participate in an academic environment.


  1. If the student is not currently taking medication related to the condition for which the student is asking for reasonable accommodations, are there medications or mitigating steps the student could take to allow the student to participate in the academic environment?


  1. Please suggest recommendations for academic accommodations and your rationale for suggesting such accommodations.


  1. Please attach any other information relevant to the individual’s needs.
