Leave of Absence, based on the accompanying Surgeon’s Certificate of Disability.

a Certificate on which to base an application for leave of absence. I do

hereby certify that I have carefully examined this officer and find that he is

suffering from gun shot fracture of left forearm, also, gun shot wound (flesh)

of left-thigh – injuries received in action, has been in hospital fourteen days and

that in consequence thereof he is in my opinion unfit for duty. I further declare

my belief that he will not be able to resume his duties in a less period than 30 days.

Regiment U.S. Colored Troops having applied

for a Certificate on which to ground an

application for an extension of his Leave

of Absence I do hereby certify that I

have carefully examined this officer and

find that he is suffering from two severe

gunshot wounds one in the left arm

producing a compound fracture of the

ulna and the other in the left thigh. (the

wounds were received on the 29th of Sept.)

I further declare me belief that he will

not be able to resume his duties in a less

period than thirty (30) days from Nov. 18th 1864.

I have the honor to

tender my resignation as Captain

in the 8th regiment U.S. Colored Troops

for the following reasons.


On the 29th of

Sept. in the charge on Ft. Gilmer I

received two severe wounds which

have rendered me unfit for service

since that time, and I cannot at

present hope to be able to return to

active service, for some time – if at

all. I have been in the service

more than three (3) years, and have

previously been twice wounded.


I am not responsible for any gov-

ernment property which I am not pre-

pared to turn over to the proper officer,

upon the acceptance of this resignation,

and was last paid by _____ _____ to

include the 31st of October.
