Job Summary
Key objectives:
Ensure consistent delivery of high-quality patient care within Peace Hospice Care Palliative Response team through effective caseload and team management
Identify through 1;1 and yearly appraisals of team core and extended skills/training needs to meet the required practice standards and role competencies
Act as a role model and expert clinical practitioner Support Peace Hospice Care policies and procedures ensuring the team are adhering to these within their daily practice
Monitor quality of care through audit/spot checks as required Work collaboratively with partners, Adult Care Services, Continuing Health Care, Central London Community Health (CLCH) and others as required
Ensure any data and required reporting is accurate and completed in agreed timescales
Ensure safe working practices are upheld in regards to training and adherence to PHC policies, guidelines and Operational procedures including Lone Working
Assist in recruitment and retention of staff
Develop and co-ordinate ensure completion of induction plans for new team members

KEY TASKS: - Some tasks may be delegated but the post holder remains responsible for ensuring that they are carried out satisfactorily.
Duties and Responsibilities
Managerial (20%)
Manage direct reports, carry out individual performance review process (annual and mid-year reviews) and support their ongoing professional development through 1:1 meetings
Perform all people management responsibilities in a timely manner and in accordance with PHC policies and procedures, including staff induction and probation process, absence management, performance management, disciplinary and grievances
With Community Services Manager recruit, select and retain staff with the right skills to deliver the Palliative Response Team
Directly line manages the Community Palliative Care Sisters within the team
Support the team in ensuring all referrals follow agreed PHC care pathways including Multidisciplinary discussion and agreed plans of care
Be responsible for ensuring mileage expenses and time sheets are completed, correct and authorised
Ensure all risk, care assessment and care plans are completed and updated by the team in accordance with agreed processes
Work collaboratively with all internal and external health care partners to ensure seamless and coordinated care for patients at all times through effective verbal and written communications
Ensure mandatory training and extended training including Clinical supervision is attended through effective and timely monthly rota planning
Investigate concerns complaints and capture compliments as needed in line with Peace Hospice Care policies and procedures
Work alongside Community Service Manager and Director of Patient Services to monitor service activity, and support ongoing development of service
Clinical (80%)
Demonstrate clinical excellence/knowledge by working as an expert clinical practitioner in palliative care
Complete accurate and timely recording of data
Undertake evaluation and audit of service and provide reports as required to support development of service
Understand the regulatory requirements for CQC
Adhere to the Safeguarding of Vulnerable Adults and the Safeguarding of Children Policies and Procedures.
Ensure agreed Mandatory Risk assessments for patients are undertaken in the home.
Monitor quality of care through audit/spot checks/clinical audit participation.
To act as a senior role model and expert clinical practitioner and work with in the Team as clinician as required
To take an active role as identified Clinical Governance extended Champion role and within the allied Clinical Governance group
