EQUIPMENT PROGRAM COMPLETING CLINICAL REVIEWS OF HOME MODIFICATIONS VERSION

Overview

Clinical reviews of home modifications are provided, when needed, to ensure that the modifications are the most appropriate for the client’s clinical needs and are being used appropriately and safely by Disability Services and Domiciliary Care adult clients. The clinician will complete a review of the home modification and the client’s situation either via a phone review, or in the home environment dependent on the actions that are clinically indicated.

The clinical review is completed by a relevant clinician when any safety or functional problem with the use of the modification has been identified by the client, carer or staff member. These problems may result from changes in the client’s condition, ability of the client to use the modifications, a change in their living situation or a maintenance issue with the home modification. These situations require clinical review which may result in training for the client or prescription of additional modifications.

Clinical reviews are completed for Domiciliary Care or Disability Services clients with home modifications provided by the Department for Communities and Social Inclusion (DCSI) when a need has been identified by the client, carer, support worker or staff member involved with the client.  The clinician completes the review with the client in the home environment and arranges referral, training, additional modifications or repair / replacement as indicated.

To arrange a clinical review of the home modification

  • Contact client / carer or other health professional involved in client’s care (as indicated) to assess the difficulty with the current home modification and the actions required.

Does client require training in use of the home modification?

If
yes, arrange a home assessment to provide necessary training (refer to procedure “Training client in the use of home modifications”)
If
no, are additional modifications required?

If
yes, go to step 2.
If
no, Does modification require maintenance / repair?

If
yes, go to step 3.
If
no, go to step 4.

To arrange additional modifications as required

  • Prescribe additional modifications or replacement modifications that are different to the existing ones, refer to procedure “Prescribing a home modification”.

Go to step 4.

To arrange maintenance / repair of modification as required

  • Fax the request to Domiciliary Equipment Services (DES), marked attention to the maintenance and fabrication officer (MFO). The MFO will provide visit and arrange necessary repairs.

If modification cant be repaired and requires replacement, MFO shall advise clinician to re-prescribe modification. Refer to procedure “Prescribing a home modification” Go to step 4.

To document that the clinical review of the home modification has been completed

  • Document the outcome of the clinical review of the home modification in the client’s file and advise referrer / service coordinator / key contact / accommodation manager of outcome of review and action taken.

  • File in client’s notes.
