Procedures for filling out Spreadsheet for Surplus Items


  1. Open spreadsheet from the Forms area on the ISDH Intranet

  2. Fill in the Data on the top of the form. Make sure you double check the “Items Located At” we have many buildings

  3. Fill in Bar Code Number (if there is one). Bar Code Number is a 4 to 6 Digit number with a bar code above it. These numbers are given to any item over $500

  4. Fill in Brief Description of item to be Surplus.

  5. Fill in Serial Number if one can be located

  6. Fill in Quantity of this item being surplus

  7. Fill in Building Location

  8. Fill in Location with the cube bar code number where the item can be found

  9. Fill in Usable or Non Usable by marking with an “X”

  10. Fill in Date

  11. Be sure to fill in the bottom line with your name and phone number


E-mail excel spread sheet to Asset Management Accountant Jean Thurnall at [email protected]


Items will then be picked up. All computers received will be taken to IOT in order to remove any information left on the computer. All items will be put in a secured storage area until which time DOA Surplus picks them up.
