(Contributor’s Name) CONTRIBUTION AGREEMENT

This Agreement, effective on the date of acceptance, is entered into by and between (name) of (address) , hereinafter referred to as Contributor and LITHUANIAN FOUNDATION, INC., a not-for-profit corporation of Illinois, with an office at 14911 127^th Street, Lemont, IL 60439, hereinafter referred to as LF for the establishment of the (name of fund) “ ” hereinafter referred to as Fund.

In consideration of mutual promises herein contained, it is agreed as follows.

Contribution. Contributor hereby irrevocably gives, transfers and contributes to LF the amount of (amount in words) Dollars (amount in numbers) ($ .00), hereinafter referred to as Contribution, which amount may be subsequently increased at any time by the transfer of funds to LF designated for the Fund which shall be considered part of the Contribution. The contribution establishing the Fund does not carry membership voting rights.

Purpose of Contribution. The Fund is created for the general purpose of .

Amount to be Distributed. The Contribution shall be invested and commingled in the general investments of the LF, subject to an administration fee of LF. Grants will be provided from the income being derived from said Contribution, as determined by LF, based upon its charitable distribution standards. As such, grants may not be available every year. Income allocated and not distributed any year may be used in subsequent years.

Grant Procedures. Granting will be done by LF using its standard procedures. The application process will be managed by the Lithuanian Foundation.

Miscellaneous. This Agreement shall be interpreted under the laws of the State of Illinois, and shall inure and be binding upon Contributor, his/her heirs, trustees, administrators, and executors, and upon LF and its successors, and may be signed in counterparts with each counterpart considered part of the original.
