Copy and paste the content below into your laboratory's letterhead.
Complete as much information about your facility as you can, print it and have a Medical or Technical Director sign it.
Fax it to 800-581-7889, marked to the attention of Password/Login help ICA_L (reference the appropriate IAC division).
Be sure to include the email address that you would like your login information sent to.

It is our strict policy to not send login information for any laboratory without a request signed by a director of the facility, on that facility's letterhead.
