All immunizations must meet Massachusetts’s minimum requirements and must be certified in writing by a licensed physician before a student is admitted to school. Massachusetts State law requires the following immunizations:
Preschool entrance requirements
DTP/DtaP/DT/Td - 4 doses
Polio – 3 doses
Hib – 3 or 4 doses
MMR – 1 dose
Hepatitis B – 3 doses
Varicella – 1 dose or documentation of chicken pox, by physician
Physical exam – within 12 months
Lead screening - most recent, date and results
Kindergarten entrance requirements
DTP/DTaP/DT/Td – 5 doses
Polio - 4 doses
Hepatitis B – 3 doses
Varicella – 2 doses or documentation of chicken pox, by physician
TB – test and results, treatment if necessary
Physical Exam – within 6 months
Lead Screening – most recent, date and results
Grade 7 - 12 entrance requirements
DTP/DTaP/DT/TD – 4 doses and 1 Td booster
Polio – 4 doses
MMR – 2 doses measles, 1 dose mumps, 1 dose rubella
Hepatitis B – 3 doses
Varicella – 1 dose under age 13, 2 doses over 13, or documentation of disease by physician
TB – test and results, treatment if necessary
Physical exam – within 12 months
These requirements apply to transferring students, as well.
Failure to meet these requirements will result in exclusion from school until written documentation has been submitted. This documentation must be translated into English.
Parent/guardian must accompany student to school the first day and see the nurse before they are allowed to enter, for pre-school to grade 8. You are not registered until the nurse sees you. High School students register with counselors and see the nurse, as well.
