T COFFEY COUNTY OTTAWA COOP SCHOLARSHIP APPLICATION NAMEGPA ADDRESS HIGH he Ottawa COOP each year generously contributes funding to the Coffey County 4-H Council for use within the 4-H program to enhance the lives of the 4-H members. The Council does that in part by offering a scholarship each year to a graduating 4-H Senior.

To apply for the Ottawa COOP Scholarship the 4-Her must:

Be completing their Senior year of High School within Coffey County with plans to further their education.
Have been an active 4-H member for the past 4 years
Have been a 4-H ambassador for the past 2 years
Turn in a completed 4-H KAP Book with this application
Complete this 2 page application with a font no smaller than 12 point.

Describe your 4-H Leadership activities and involvement.

What significance has the 4-H program had in your life?

What are your future plans? ie. What are your plans for further education and your career goals?

What would you tell a youngster to encourage them to participate in the 4-H program and what do you see as a role of 4-H for the future?
