An Order Set is a pre-printed or pre-formatted physician order and is developed for a number of reasons including:
Improved safety and error reduction |
Improved work flow |
Better compliance with best practices |
Other persons who may order an Order Set are Acute Care Nurse Practitioners/Midwives
What do staff need to know?
In addition to date and time, a physician signature is required at the bottom of each page of the Order Set
The following table provides a description of the symbols you will see on the Order Set that will impact order transcription

These symbols give the physician flexibility to adapt the order set to individual patient needs
Only those who have ordering rights can order on an Order Set unless the physician has given a telephone order for an order set to be initiated, then:
The RN may initiate the ordered pre – printed set and tick in the option boxes ONLY if the physician has ordered
Remember: always read back those ‘tick box’ orders and any defaulted orders that the physician has decided not to order
The Physician must then verify those orders on the next physician visit
Once the orders are signed – no one can add, subtract or edit those orders. If a change order is to occur then the Physician (or in the case of a telephone order, the RN) will write beneath all previous orders:
A statement of either clarification to the ‘identified’ order OR a discontinuation of the ‘identified’ order
Write new order
Send copy to Pharmacy (if applicable)
A Clinical Protocol is an ‘order set’ that contains all default orders. Default orders are recognized by the bullet symbol preceding the order
Can be initiated by ordering the name of the Clinical Protocol itself
Can be ordered as part of a pre-printed order set or as part of a patient’s on-going treatment plan
Unlike preprinted Order Sets, a Clinical Protocol does not need to be signed by the ordering clinician to be implemented (but does need to be ordered to activate)

Searching for and Printing an Order Set or Clinical Protocol on your unit
Click search feature on top of the page, then
Type the order set or clinical protocol title in the first field
Enter search, a list of order sets or clinical protocols containing similar names will appear on the screen
Click on the title to open the document
Or by:
Click Advance search and enter the detail you wish to use for your search eg: Corporation option has a drop down box for GBHS, SBGHC and Hanover
Or by:
Clicking on the Health System listed on the left and then the drop down options within that health system (e.g. General Medicine, Oncology, ect)
To print copies:
Click on the Printer icon at the top of the file you opened
If the Clinical Protocol is 2 pages or more double sided print option is available on most printers.
