PROGRAM MODIFICATION FORM

Effective September 13, 2018


All proposed changes to approved programs should be noted in the following form. Should these changes be deemed nonsubstantive, the Office will give notice to the institution and note the changes in its records, with no further action by the institution required; should these changes be deemed substantive, the Office will review the changes as a program modification according to procedures outlined in section 10a-34-3(c) of the Regulations of Connecticut State Agencies.


According to section 10a-34 of the Connecticut General Statutes, all new programs, program modifications, program accreditations, new institutions, and institutional accreditations must be approved by the Office of Higher Education. Each proposal for a program modification will be reviewed by the Office, and the institution may be asked to provide additional information. Should the office determine that 1) the modification is substantive, and 2) that the modification is in compliance with the standards as defined in in sections 10a-34-9 through 10a-34-24, inclusive, of the Regulations of Connecticut State Agencies, then the modification shall be deemed approved within 45 working days and no other action by the institution shall be required.
